PETS Record Form Required for Butler County Fair

4-H'ers exhibiting/showing pets at the Butler County Fair will complete a Pet Record Form as part of their project exhibit. In addition, youth exhibiting/showing cats or dogs will complete a section on the form verifying completion of yearly shots and have a veterinarian signature verifying the information is complete and accurate. These changes have been made to ensure the safety of the judge, exhibitor, and the viewing public. Click on the link to print a record form.

After you click on the following link, you can complete the form online, then print the completed form from your computer. You will need to sign the form and make sure your veterinarian signs it also.
